What is complex care?
Complex care is specialist support for people with one or more long term or chronic health conditions, particularly where one condition influences the care of another. As part of this support, C4 Care manages a complex care plan that coordinates a team of healthcare professionals, improving an individual’s experience of living with complex health needs.
Everyone in our complex team receives specialist training to ensure they deliver the very best care. They will manage symptoms and work with healthcare professionals to deliver person-centred complex care at home. We focus on compliance, compassion, and wellbeing in day-to-day care to enable highest quality of life and independence
Who is it for?
Complex care supports anyone with a chronic illness, disability, or long-term condition and who needs a lot of additional support. Examples might include stroke recovery, dementia, mobility disorders, Parkinson’s, depression, brain injuries, or neurological conditions.

- Supporting those with specific medical or health conditions
- Communicating with others using aids or support
- Behaviour management
