What is Complex Care?

Complex care is specialist support for children and adults with acute and/or chronic health conditions that may have complex needs as a result. As part of this support, C4 Care can build a care plan that takes every aspect into careful consideration and provides the right level of support that meets all requirements and provides caregivers with a high level of expertise and training in those areas.

Every caregiver in our complex team receives specialist training to ensure they deliver high-quality care. They will manage symptoms and work in collaboration with the multi-disciplinary team to ensure person-centred care in the service user’s home. We focus on compliance, compassion, and wellbeing in day-to-day care to support the highest possible quality of life and encourage independence.

Which services do we provide?

Vital signs include measurements such as heart rate, blood pressure, respiratory rate, and body temperature. Vital signs monitoring is a crucial aspect of healthcare to assess a patient’s overall health and detect any abnormalities.

Ventilation involves the mechanical support of breathing, often through the use of a ventilator. Care includes ensuring the ventilator settings are appropriate against the prescription, maintaining airway patency, monitoring oxygen levels, preventing infections, and providing regular oral and respiratory hygiene to support respiratory function and overall health of the individual.

A tracheostomy is a surgical procedure that creates an opening in the neck to insert a tube into the trachea, allowing direct access to the airway for breathing support. Our care involves cleaning and maintaining the tracheostomy tube and surrounding area to prevent infection, ensure airway patency, and promote healing.

Suction care involves clearing an individual’s airway by removing secretions, mucus, or fluids from the mouth, throat (via tracheostomy) using a suction device, helping to maintain airway patency and prevent respiratory complications.

Enteral feeding is a method of delivering nutrients directly into the gastrointestinal tract through a tube, typically used when a person cannot eat by mouth but has a functioning digestive system.

Catheter care involves the cleaning, monitoring, and maintenance of a catheter and the surrounding area to prevent infection, ensure proper drainage, and promote comfort and hygiene for patients with urinary or other types of catheters.

Bowel care encompasses activities and interventions to maintain the health and regular functioning of the bowels. This may include strategies to address constipation, diarrhoea, or other bowel-related issues in individuals with certain medical conditions or limitations.

Stoma care involves the management and maintenance of an opening (stoma) created surgically on the abdomen to divert the flow of urine or faeces from the body, typically after procedures like colostomy or ileostomy.

Palliative care is specialised medical care aimed at improving the quality of life for individuals facing serious illnesses. It focuses on providing relief from the symptoms and stress of the illness, rather than curing the disease itself.

Our specialist team are managed and supported by adult and paediatric nurses with over 20 years of health care experience, and work together to provide highly coordinated person-centred care for people with complex health and social needs to achieve better health and well-being. They are trained in life sustaining treatments such as oxygen therapy, ventilation, bowel and bladder management and chronic symptom management for pain and nausea.

As a team we will work collaboratively to understand your needs and preferences in order to achieve your goals, in addition we will work alongside the multi-disciplinary team to collaborate a support plan that is unique to your needs. Our holistic approach means we will support you in all areas such as clinical task, medication, personal care, hobbies, companionship and where required maintaining your home.
