What is Outreach Support?

Outreach support means providing floating support for anyone who might need assistance with everyday tasks around the home, personal care, and daily living. Outreach support is used by people of all ages both long-term and for short periods.

Our care plans are flexible and depend on your requirements, from providing a few hours of support a week to more intensive periods. The main goal of our care workers is always to promote confidence and independent living.

Our community and outreach support staff can provide floating home care support should you need it, providing help with everyday tasks.

Outreach Care
Helping to provide support, skills, and confidence for independent living.

Companionship
Community support for people with a variety of conditions and injuries who need additional help.
We work with people of all ages who need support in their homes and daily life. Some have temporary care needs such as those recovering post-op or experiencing an ongoing medical issue. Others may require long-term support such as those with learning disabilities, mental health, or mobility issues.

Which may include:
  • Dressing and personal grooming
  • Personal care and hygiene
  • Medical appointments and taking medication
  • Light housework
  • Meal preparation and nutrition planning

Since 2018, C4 Care have been providing community care & outreach support services on both a temporary and permanent basis. Our services are staffed by community care assistants and support workers with enhanced DBS checks, training in Positive Behavioural Support and Level 2 Safeguarding at minimum. All our team benefit from ongoing training and frequent refreshers in both care skills and specialist treatment.
